Each person got green dots and red dots. Green meant "this looks like health to me." Red meant "this looks like wellness." We never defined either word — that was the point. Across 36 images, they placed 435 dots.
This isn't a like-or-dislike study. Both colors are honest categories, and the question is where people draw the boundary between two ideas our language treats as twins.
No definitions were given — and yet the sorting was anything but random.
Health turned out to be the measurable body. Exercise, data, doctors, and actual food. The images that went green are full of effort and evidence: a sprinter wrapped in pace graphics, a workout mid-rep, a clinician in gloves, wet vegetables, berries with nutrition percentages.
Wellness turned out to be the felt body. Rest, touch, ritual, and atmosphere. The images that went red are quiet and receptive: a massage, a yoga stretch, dew on a leaf, soft abstract shapes — and, strikingly, nearly every product on the boards.
Health is the body doing and being measured — effort, data, medicine, food. Wellness is the body being cared for — rest, touch, atmosphere, and almost anything sold in a package.
The sharpest pattern in the study sits on the food board.
Every image of actual food leaned health: the vegetables (88%), the blueberries (78%), the botanical orange (4 of 4 dots). Every packaged product leaned wellness: six supplement and label designs went red, two of them unanimously. The protein powder — a product that talks like nutrition — split the room.
Read those two together: it's fruit both times. What moves the image across the line isn't the food — it's the packaging. The moment nutrition becomes a branded product, people stop filing it under health and start filing it under wellness.

Both images are built on data. But people read measurement as health and marketing claims as wellness — even when both come wrapped in percentages. The number itself isn't the signal; it's what the number is doing.
Seven images split the room — and they map the exact seam between the two ideas.
The rest of the middle tells the same story: a grain bowl (styled food), a gritty gym poster (effort as branding), stylized fruit icons, greens laid out in petri dishes, a molecular close-up. Each one mixes a health cue with a wellness treatment — and the votes split almost exactly along the mix.
Across all three boards, the sorting followed one deep pattern. Health is something you do — run, lift, eat, measure, treat. Wellness is something you receive — a massage, a ritual, a product, a mood.
And there's an economic edge to it: on these boards, nearly everything that could be purchased read as wellness, and nearly everything that had to be done read as health. The public seems to have quietly absorbed the idea that wellness is an industry, while health remains an activity.
People sort the two words by agency and evidence: health is effort you make and numbers that measure it — wellness is care you receive and products that promise it.
Neither category is better. But knowing which one your image signals is the difference between being heard as medicine and being heard as lifestyle.

“Health” and “wellness” circulate in public discourse as near-synonyms, yet policy, regulation, and marketing treat them very differently. This study asked whether lay audiences hold a stable distinction between the two — and if so, what visual cues carry it. Twenty-six participants sorted 36 images across three themed boards (Medical & Holistic, Food & Supplements, Fitness & Treatment), assigning each dot to health (green) or wellness (red). Because both poles are legitimate categories, scores below are reported as category shares, not approval: an image at “100% wellness” was not disliked — it was unanimously classified.
Reading note: dot density reflects attention as well as classification — the petri-dish greens drew the study's largest crowd (24 dots) and split it, while several unanimous verdicts sit on small counts (4–11 dots) and are flagged accordingly in §8.
Given no definitions, 26 participants produced a boundary consistent enough to describe as shared cultural knowledge. Health collected the active and the evidentiary: exertion (the pace runner, 11/11; the tennis swing, 4/4), measurement (the gym interface, 86.7%; the run tracker, 78.6%), clinical authority (the gloved clinician, 88.9%; the DNA helix, 7/7), and whole food (fresh vegetables, 88.2%; annotated blueberries, 78.3%). Wellness collected the receptive and the atmospheric: massage (89.5%), editorial calm (93.8%), dew and gradient abstractions (85.7% and 6/6) — and, most consequentially, nearly every packaged product on the boards, including two unanimous classifications (NEUROBOOST, 11/11; the pomegranate label, 6/6).
The public boundary runs along agency and evidence. Health is what the body does and what can be measured; wellness is what the body receives and what can be purchased. Productization was the single strongest wellness cue in the study — stronger than any aesthetic property.
Two second-order findings sharpen the picture. First, data is not one signal but two: numbers that document the body (pace, elevation, daily intake) sort to health, while numbers that promise an outcome (“89% said their skin was smoother”) sort to wellness at 72.7% — the form of a statistic matters less than its rhetorical job. Second, aestheticization moves images across the line: medical imaging reads as health in a clinic but drifts toward wellness once rendered in golden glow (the ribcage, 66.7% wellness) or fused with athletic art direction (the x-ray runner, an even split). The boundary is not about subject matter; it is about treatment.
Twelve images crossed the 60% threshold toward health. They organize into three families: effort, evidence, and food.

The medical establishment's iconography sorts green with near-unanimity: the DNA helix took all seven of its dots, and the icon pattern and gloved clinician each posted 88.9% on healthy crowds of 18. The cell illustration (70.0%) sits lower — a first hint of the aestheticization effect examined in §4: as scientific subject matter becomes decorative, certainty erodes.

Unprocessed food is health: the wet vegetables (88.2%) and the field-guide orange (4/4). The blueberries (78.3%, the board's largest crowd at 23 dots) add the study's clearest evidentiary cue — fruit annotated with daily-intake percentages reads as nutrition science, not marketing. The lower share relative to the plain vegetables is worth noting: even helpful annotation is a layer of mediation, and every layer costs a few points of health certainty.

The board's health pole is exertion under measurement. The pace runner is the study's cleanest health verdict — 11 of 11 — combining full effort with performance graphics. The tennis swing (4/4), instrumented yoga session (86.7%), night-run telemetry (78.6%), and dumbbell work (72.7%) complete a family in which the body is visibly working and the work is visibly counted. Notably, the two interface screens in this family sort green because their data documents activity — the contrast case arrives in §4.

Board 1's red pole is what might be called science's aesthetic echo: the same subject matter as the health cluster, rendered for mood. The gradient orb (6/6) and the mosaic of faces (8/8) are unanimous; dew on a leaf posts 85.7% with no body or product in frame at all. The three most instructive entries are partial migrations: a touch-based interface about microbes (72.2% wellness — an app, but its data is skin-deep and sensory), an x-ray torso in golden glow (66.7% — medical imaging aestheticized), and capsules staged in a petri dish (60.0% — the laboratory restyled as a spa). Scientific content does not anchor an image to health; scientific treatment does.

Six branded objects, six wellness verdicts — two of them unanimous (NEUROBOOST, 11/11; the pomegranate label, 6/6). The gradient is instructive: the more lifestyle-forward the packaging (violet neuro-capsules, boutique juice, engraved spice branding at 85.7%), the more decisive the classification, while the more clinical-adjacent products (probiotics with a “dietary supplement” panel, 61.5%) sit closer to the line. Nothing in this family was disliked — it was categorized, and the category was consistently lifestyle rather than medicine. Section 7 takes up what this implies for nutrition communication.

The treatment pole mirrors the effort pole point for point. Where health's bodies work, wellness's bodies receive: massage under benefit copy (89.5%), the BALANCE editorial (93.8%, 15 of 16), the yoga-mat stretch (68.4%), the maternal silhouette (60.0% on five dots). The skin-statistics panel (72.7%) completes the data contrast opened in §3: percentages in service of a feel-good claim sort red even though their visual grammar — circles, figures, serif restraint — is identical to legitimate measurement. Participants read past the form of evidence to its function.
Seven images failed to reach 60% in either direction. They are not noise — they map the seam where the two constructs overlap.

Every ambiguous image mixes one cluster's subject with the other cluster's treatment. The petri-dish greens — whole food arranged as laboratory specimens — drew the study's largest crowd (24 dots) and split it 46/54: the produce says health, but the art-directed lab staging carries the same aestheticized-science signal that pulled the capsule dish (60.0%) toward wellness. The protein jar (41.2% health) is the commercially significant case: a product that speaks nutrition's language while wearing wellness's packaging, and the room divided accordingly. The x-ray runner (44.4%) fuses clinical imaging with athletic art direction; the TRAIN HARD poster (an even 4/4) renders exertion as branding; the grain bowl (55.6%) is food under styling; the fruit icons (58.3%) are produce abstracted into graphics; the molecular macro (2/2) is science as texture. The middle, in short, is where treatment fights subject — and the vote records the stalemate.

Classification strength, in rough order: (1) productization — packaged goods sorted to wellness in every appearance, twice unanimously, making commerce the most reliable single cue in the study; (2) bodily agency — acting bodies sort to health, receiving bodies to wellness, with no exceptions above threshold; (3) the function of data — documentation sorts green, persuasion sorts red, independent of visual form; (4) aesthetic treatment — glow, softness, and styling migrate any subject toward wellness by 20–40 points; (5) attention predicts nothing about direction — the study's largest crowd landed on its most evenly split image.
One interpretive caution: the boards cannot distinguish whether participants sorted by concept (health vs. wellness as ideas) or by industry recognition (the look of the healthcare system vs. the look of the wellness economy). Both readings fit the data, and both may be true — the wellness industry's visual language is itself a large part of what the word now means. The follow-up designs in §8 would separate the two.
Neither category is superior; the risk is signaling one while intending the other.
Policy-adjacent note: the supplement result deserves particular attention. Products that consumers purchase as health interventions are visually classified by the public as lifestyle goods — a perception gap with plausible consequences for how claims on such products are weighed, trusted, and regulated in the public mind. Conversely, public-health campaigns that adopt wellness aesthetics to feel approachable may be trading away the very authority that makes their guidance read as medical. This study cannot settle either question, but it locates both precisely on the health/wellness boundary.
Twenty-six participants (100 opens) placed 435 dots across 36 images on three themed boards of twelve (147 dots on Medical & Holistic, 146 on Food & Supplements, 142 on Fitness & Treatment). All boards ran an identical bidirectional prompt assigning green to “health” and red to “wellness,” with no definitions supplied. Images were classified at a 60% share threshold; images below threshold in both directions are reported as ambiguous rather than discarded, since in a bidirectional design an even split is itself a finding.
Confidence varies with crowd size. Per-image counts run from 4 to 24 dots; the unanimous verdicts on small counts (the DNA helix at 7/7, the tennis swing and botanical orange at 4/4, the gradient orb and pomegranate label at 6/6) should be read as strong directional signals rather than settled facts. The findings that support weight-bearing conclusions are convergent ones: the productization effect repeats across six independent branded objects; the agency pattern (acting vs. receiving bodies) holds across every above-threshold image on the fitness board; and the two-faced behavior of data replicates across four separate interface or annotation images. At n=26, all percentages are directional; the architecture of the boundary is what the study establishes.
Suggested next passes: a matched-pair board isolating productization (identical fruit, photographed loose and as a branded package) would convert the study's strongest claim into a measured effect; a demographic split would test whether the boundary shifts with age or with supplement use; and re-running the boards with the prompts reversed (green for wellness) would rule out any anchoring effect from color valence. A definitions-first variant — asking participants to write one sentence on each term before sorting — would separate conceptual sorting from industry-recognition sorting, the central interpretive question this design leaves open.
